She doesn't have an official name in the show. Fans started calling her Derpy Hooves when they spotted her in the first episode. "Ditzy Doo" is mentioned in "Fall Weather Friends" as the name of an unseen pony with a poor sense of direction, which Lauren Faust suggested on DA could be Derpy's actual name, but she left it open.
I like to think her name is Ditzy Doo, and "Derpy Hooves" is a nickname.

Rainbow Crash is at least one. It's based on the regular name, but it is distinctly different. And Spike gave them all names while they were 'cursed'. Spittie Pie, Hairity, Flutterguy, Appleteeny/Appletini... I think the one for twilight was Twilight Flopple? I forget.
And I could easily see ponies calling Soarin PieGuy or something.
Isn't Pinkie's name even a nickname? Pinkamena being shortened to Pinkie. I guess it's sort of like your last example, but people use it all the time. Less of a 'pet name' that way.
