Maybe everything was going ok, people held up their hands, you got bags full of money and your crew are exiting the bank. Only when you get outside, standing before you is one of the Heroes you've heard about. "Hey there guys, you wanna put those bags and guns down before somebody gets hurt? I'll only ask nicely this once~" and through panic, stupidity or arrogance they raise those guns instead. It's a shame, they could have been sent to jail without some broken bones but some just want to do it the hard way.
