A commission I got from 
Dan_the_Bear several months back of Jam swapping heads with Rafiki. :) Looks like Rafiki wanted to be young again for a bit, and decided to use Jam's body to accomplish that. In the process, he made sure to age up Jam's head to fit his older body! Now Jam's wrinkly and missing some hair and teeth! He'll be back to normal once Rafiki gives him his body back... Probably!
