I'd like to think I'm reasonable enough that so long as they're both consenting adults and aren't total idiots about it, I'd do my best to accept it.
But seeing as I don't have kids and don't know how that would change my outlook, I can't say for sure.
I know the question wasn't pointed at me but I thought it would be fun to answer regardless.

Despite not having children either I would have this odd predisposition towards accepting it if they were brother/brother or sister/sister, simply because the whole pregnancy aspect doesn’t sit too well with me.

I asked this question to a co-worker who has kids she said she feels like a failure as a parent. I think you will get different answer depends on who you ask mom or a father.
My answer be I wouldn't mind but I would more worried about what others people think if they found out.

I don't know what I'd do. I'd probably be very hurt and disappointed in them possibly angry. Then I'd start questioning what it was I did wrong as a parent. That's some of what I'd think and feel but what I'd do I honestly don't know.
Now obviously this comic is comedic/erotic. In my opinion for the most part it should stay reasonably light and not dwell for too long on heavier stuff. Besides, there can be comedic potential in the whole almost getting caught shenanigans, having to explain why you have a baby, heck even the birthing process can be played up comedically as well.
Anyway, my two cents for whatever its worth.

Morning sickness can sometimes last all nine months, and in some pregnancies it can come and go randomly through out. I had an ex who was fine as long as she didn't think, smell, see, taste or feel eggs during her pregnancy, otherwise she would end up yacking up her last meal and this was when she was showing as much as Rina was. But then in this case for Rina it could be a mix of pregnancy and motion sickness.

I've recently learned that some species can terminate pregnancy after the alpha male is killed or exiled because the new alpha will kill the offspring and it's a waste of mother's resources to carry them.

Having sex is not advisable in late stages of pregnancy because it can trigger the childbirth process in inconvenient circumstances (don't do it in a week long trip to the mountains where ambulance won't reach you)
Sometimes, it's the opposite, doctors advise you to have sex to deliver the baby quicker it is necessary.
