Life hasn't been good to the aging rat, and he hasn't been good to it in turn, having built a reputation of fear and loathing across the world of his time. In a rare moment of reflection, the scarred and weary killer perches himself in the stone hands of a statue of his venerated Hygiea and thinks about what is needed but missing in his life...

It's rare to see the old Rat in a state of regret and contemplation.
So often he's riding high on unrepentant villainy or deep in delusional denial.
The fact that the statue is so cold and unresponsive probably doesn't help his condition, and it all probably ends with him protectively curled up in the palms of his silent goddess, forced to suffer until his fractured mind can construct more walls of denial to let him function once again.

A sound and probable analysis! Of course, it is possible that the peaceful repose - so different from his everyday life - coupled with the boundless compassion expressed by the statue's chiseled countenance leads old Sandoval to some beneficial conclusion and he learns to conceive and function better... Time will tell!
I'm glad you like this, and can notice the skill and effort put into it... I think his most direct reflection here is that he's done almost everything possible for other beasts to hate him and only has his religion to turn to, and now even this isn't providing him fulfillment.

It's Hygiea! She's the goddess of health. The bowl and snake she's usually seen with are modern medical symbols.
She wasn't my first choice - Athena would definitely have been more in-line, here - but like I said, trying to find a statue in this pose is a pain, and Sandoval venerates all goddesses in general.
