The only rules to putting the icon in your profile are these:
1. You must be a trained healthcare provider (OR)
2. A student training to become a healthcare provider (OR)
3. Personnel currently employed in a position of vital support for the diagnosis and treatment of patients.

Example of vital support: lab technicians that don't have patient contact but that identify diseases in biological samples
Example of non-vital support: clerical work such as submitting patient insurance for billing, janitorial work in hospitals, or greeting patients at the door, etc.
