Recently i opened for commissions and i am not seeing a whole lot of activity. I would like to know what you guys think?
Is the form to complicated or is the prices not okey ? Or is there anything else that is a turn off?

I can't say I speak from any long term experience with you directly, only came across your work yesterday while searching for commission openings.
Speaking from a customer viewpoint, the hourly pricing is a bit intimidating at first glance. Unless you're an artist, you don't have much of a framework to go on as far as how long a piece takes. It seems easy to just ask for a quote, but just getting to that point is usually the biggest hurdle because it involves the fear of the social interaction afterwards, worrying you will be quoted a price that's beyond your budget and you will feel like you wasted both of your time or feel pressured to accept it, even if no external pressure is being applied.
I think the simplest solution to that would be to include examples of works and their time to completion in the pricing, so people have a rough estimate of what they would be looking at.

I have since updated the form and give a few samples that make it easier also not that asking for a quote is completely free and the quote price is final.

2. When the customer does get an impression of prices, it's hourly, which is usually much less desirable than actually being able to know outright how much a pic will cost unless it's on the cheap end, which you aren't

I do agree with a point that was made that the hourly pricing can look intimidating to a commissioner when many other artists do flat price sheet. That being said, you are very up front in your form that asking for a Quote is totally free, which I think is great. In your previous Journal, you mentioned that the Quoted price is final, but I don't see that mentioned in the form if this is still true. If this is still applicable, leaving that out of the form may give the impression that there could be a "surprise" bill at the end. I think the examples you do provide are helpful as guidance. People just need to be more okay with asking for a Quote.
In regards to prices, you've already lowered them since opening the form, and you know how much your time is worth. I saw a similar situation with another artist where they finally started taking commissions after a long while and it did take some time for people to "rediscover" them as someone to commission. Eventually their openings grew shorter because of increased demand. And while there may be "cheaper" artists out there, it sometimes shows with a lack of professionalism or even getting the artwork at all (speaking from experience).
