The only recommendation I have is on the blood. You did a great job in shading it and highlighting where the light were show through, but you did put the light on it. (I'll draw up a quick sketch to illustrate what I'm talking about.) ;)

The barbed wire is an excellent touch. I love the scar on the top right and how you can see where the stitches were once.

I am a little confused about what it is you mean, exactly, but I'll see what you mean when you sketch it. (:
