The rattlesnake is about to bite a worker, and no one sees the snake
There are people without proper safety helmets and vest,
a few are lacking eye protection around the welder,
a guy is paying attention to something in his hands instead of where he is walking
The driver of the forklift is also distracted while operating it

ANNND, the one dude is throwing the switch to change the track and the other that he's supposedly looking at, has his foot between the tracks getting switched.

And the switchstand is not at the switchpoints

....and they all need to go on a diet!!!! ;- )

I could easily be wrong about this las one but...
I'm not sure what the deal is with that signal mast in the background, but I'm sure it should be as tall as the locomotives and have the signal heads mounted near the top.
