An old Canon Rebel XT, a crappy Tamron 28-80 aspherical lens, a 2x teleconverter, Norman 808m studio light chargebox, two LH500+ lampheads, one 32" gold tone umbrella. a 40" rectangular soft box, 28mm zoom, (56mm with extended depth) ISO 100, Aperture 5.6, shutter speed 100

You can pick up a camera like mine for like 80 bucks now, and the lens I used was 30, the lighting is a lot more but you can totally improv that with regular lights, and the teleconverter isn't really needed unless you want to shift the focal distance for more depth of field, in this case I could have just blurred it out on my own with photoshop but I don't like changing photos too much from the original shots so I got all picky with it.
Most any DSLR or decent point and shoot will do, but the key is to have a crapload of light on the subject so that if you are using say, a cell phone or (my phone has a higher resolution then my dslr and it's a trackphone, go figure) or any automatic camera, if you have a lot of light, the ASA or "film speed" will be lower, allowing you to get nice clear, non grainy shots, plus composition of the subject and position of the lights help too but the main thing is to get enough light to use the lowest possible film speed and shutter speed.
