The outlines are the top layer.
Basic coloring is the first I start to work with after the outlines, the shadings are all seperate layers above the basic color, same goes for highlights.
Outlines stay on top all the time, works better if you decide to color them after you're done with the rest.

I usually keep my outline layer on top at all times as well and then would but a base colour underneath, but the shading above and the highlights above it. I was weird though and for the longest time I'd do my shading on the same layer as the base colour but split the highlights. XD No wonder it'd take me so long, especially if I made a huge mistake.
How do you color the lines though? I noticed the outline seemed really nice and crisp. Usually I just scan in pencil work and clean it up, but now I'm getting in the habit of using the Pen Tool in Illustrator to get those lines, but I haven't actually tried recolouring them. Is it just a simple click and fill?

If one of my pictures has colored outlines, I pretty much scanned the scribble and drew the actual outlines on Photoshop.
Clean outlines are easy to do on a computer, even if your hands are a little shaky: Zoom in to 200% (300% if some details require it) and just draw. You zoom out to normal size, you'll have nice, clean outlines.
