This is a penanggalan, a creepy-ass Malaysian vampire that's basically an entrail-trailing head that flies about at night. Depending on the version of the legend you get, sometimes she strangles people with her intestines, sometimes her hair gets into the act, and sometimes there's friggin' MOUTHS on the ends of those things. The symbol in the corner is one of the alchemical symbols for vinegar, in which the penanggalan sometimes soaks her organs so they can squeeze back into the neckhole of her body come daybreak. Folklore is weird.
You can keep them out of your house by putting thistles around the doors and windows, which snag on the danglin' guts and upset them most highly. Sometimes you need to use briars grown from the penanggalan's own poisonous ichors instead. Their favorite food is the blood of babies (especially babies), children, and pregnant women. Seriously, Google these things, they're wicked-awesome.
I took plenty of artistic liberties with anatomy here, but I think it kind of adds to the yacky unreality of the picture. Tree root guts for the win?
