He's freakin out because there is a spike coming out of the middle of his chest and he thinks someone just impaled him with it, hence his total freak out in the last panel.

Basically, yeah. -.-; Based on who the artist is, the could be a bit more meaning behind it than otherwise, but the surface joke is all that's really needed for it to be funny, IMO. ^_^
