Mew was added last minute. After the debugging tools were removed from the ROMs, there was just enough space for one more Pokemon. But, Mew was never officially planned to be a part of the game.
Pokemon just don't die in-game. In Sinnoh folklore (at the library), it mentions picking the bones of a Pokemon clean. That, and various towers (like Sprout Tower) mention something about departed Pokemon.
