Spidey has had several design variants from comic to comic. I'm sure i've seen at least one with the large eye patches.

The white part of his eyes are smaller to show that he's giving her lip. It's not a design change, just a matter of style.

Depends which Spidey it is...and who's drawing him.

From the looks of things, it's hard to show off Spiderman's facial espression when it comes to drawings. I think what he wanted to do was show off his facial espression (while at the same time, show off how spidey's eyes are by default). If you was to invision the black markings as white, you'd see what I'm talking about.

I think this was the best option he could go with (at the time).
