I've thought FA was doing that too...
But what I've found out is that if its the same filename (and maybe it doesn't even have to be that), the cookies FA stores during submission will show a resized thumbnail that makes it look like you're submitting the wrong file. Once its submitted, even looking wrong, I find that if I ctrl+f5 the page it really HAS been changed.
The structure of history on this site is just sorta... fumbly.
