This view might give you some idea of how you "enter" it. The parts where the king-size pink bed sheet are tied act as stirrups for your legs/feet should you choose to do so. I typically do since it helps keep the thing together, plus it increases the sensation of helplessness.

But of course, you need to be able to "evac" with relative haste in case of any sort of "emergency" (typically of the "bathroom" sort). It's not too hard to wriggle into or out of it.
