No fur is quite sure where the practice of giving individual ships insignias happened, nor was it ever really formalized. But there were plenty of superstitions about it. In the Federation, a ship is never launched without a name. However, that ship never earns their insignia until they've completed their first mission, and ideally after being "blooded" by defeating an enemy. Only after the ship has returned to its home port is the design process begun, and it is an internal affair until it is placed on the ship itself, and thereby registered with the Navy.
