I really feel for the poor guy. He has a lot of body image problems and is probably too scared to admit it to her, given he assumes she thinks less of him, wants some petty revenge over a relationship and thinks she secretly hates him or something. She, for one needs to be more transparent about her true intentions.
Sooner or later, he’s gonna crack under pressure and she’ll have to be honest about whether she truly wants to help him or not.
The truth can either set one free or trap them further. Both can easily apply and he would have a right to walk away because if she was being vindictive and acting under false pretenses, he has a right to be pissed about it.

what i would do?
i would ask abe (my boyfriend) if he could just keep an eye out for anything "untoward" going on:
be it from ruby, or be it from bystanders
i understand where basil's coming from, of course:
having been (and, in many ways, continuing to be) a favourite target of bullies, myself
easier said, than done, of course:
that, if someone really wants to get your attention, with their abuse, they'll do just that
if they're too cowardly to address you directly, then that's all the better for you
on the whole, of course:
most people, i'm sure, are just trying to get on with their day — and if they do pay you any attention, it's only because humans are curious animals who, by rights, always want to learn about their environment
it's almost never anything personal to you, specifically
but, if someone wants to make you their business..
..normally you're empowered to give it right back to them
if this is coming from someone who is clearly Emotionally / Psychologically Disturbed, you don't engage with them — they don't have any regards for themselves, anymore than they have any regard for you
