I'd think the general beats would be:
April enjoying the feeling of being a Sumo. As would the Turtles.
Shredder would probably have a counterpart (possibly a 1987 version of Karai, likely using the Shredder's Revenge design) to fight against April.
There are more ideas, but they'd push beyond the age rating assuming this idea did become an episode of the 1987 series.

Outside of the two I'd mentioned already, there's:
The Frenchfry episode from the Lilo and Stitch series, the Passion Patties episode from Totally Spies and maybe the Human Bender bit from Futurama. Those are the ones I immediately think of. There is another Futurama example, but that falls more into inflation than weight gain.
