It may be hard to believe, but that is in fact how his neck usually is, just a massive stack of soft fat-filled rolls towering multiple feet in the air. As you'd expect it makes everyday things like eating normally and looking downwards quite difficult, not to mention the fact that balancing several hundred pounds of neck fat on his shoulders makes waddling around even harder to do without falling over. At least its extremely comfortable though.
