Probably wouldn't be too hard...
Extend the bottom of the sky 'till it's a 9.5:12 ratio (1000px * 12/9.5 = ~1264px), double it up mirrored (I'd put the rainbow towards the spine on both the front and back)), add something upside-down to the bottom that matches to form the inner pocket (about 4" worth is good), maybe mirror the edges and top into a small amount of "bleed" area around the edges (any extra can be folded into the folder to strengthen the edges), and have it printed onto heavy card stock someplace. (the total area not including folded over "bleed" area but including the pocket would be 19"x16". An extra 3/4" on each side and on the top for the bleed area would be good, bringing it to 20.5"x16.75")
if it's way over on inches at whatever the biggest size you have it at is, increase the DPI to fit it rather than decreasing the pixels. Less loss of quality that way, and with printing, more DPI should be better anyways.
(If you're wondering where I got the inch dimensions and construction details from, I used a ruler on an old Lisa Frank folder I had laying around. Maybe grab a cheap not-plain folder from Wal-Mart to copy the construction of.)
