Usually how my dm does it, but he made you roll after the 1 to determine severity, but it could be something silly or simple like tripping or having your bow string snapping and popping you in the face. If you rolled a second 1 in that roll you rolled again, chances are it's going to hurt a lot. If you rolled 3 nat ones then it wasn't meant to be and your character is killed in some way.
One session in particular was memorable. I was the DM and the party was dungeon crawling in an abandoned underground city.
They came across a hidden locked chest when they failed to open it with a crit fail. The Borderlands 2 Dragon Keep DLC just came out a few months ago and I couldn't resist. I said to the guy who failed: "You fail to pick the lock. Out of frustration, you try to brute force the chest open and break several fingers as a result. -3 Dexterity and -10% max health until you return to town."
We had another rule that unless its something like a curse (or stated to specifically be permanent/relied on equipping an item), all status effects (both good and bad) are removed upon returning to the town where the adventure started.
