I basically just sewed the shape of the horns themselves, and then took some string and wrapped it around the horns for the indented look. I fastened it in some hidden places so that it doesn't unravel too, but that's the gist of it.

I usually don't, since once I make a hat or plush, it's really easy for me to re-create it. I've never really found it necessary, since I just remember everything. :)
