There's orange and black for normal inmates, and some get the death-stamp / death-mark on the forhead (55046 never did).
There's blue and black for 'the worst offenders' and they always get the death-stamp and are done away with.

I don't know. The artificial organs were a lot of cost (valuable prototypes) and effort to install. But I wouldn't be surprised if neutering and nullification was more common (if not already). It's one way you could punish inmates already in such a harsh prison. "Off with his balls!"
