Yes, that is a real cigar box! I was actually looking for a cigar box guitar (CBG) made by a different company which featured a drawing of a grizzly bear on the box. The bear CBG is no longer available, but when I found this one on Etsy, I knew that it was just right for me.
This one-of-a-kind instrument was built by hand by Charlie Rehfeld of the Tejas Cigar Box Guitar Company in San Antonio, Texas.

Only 4 strings, but not a bass. Cigar box guitars (or CBGs) are most often strung with either the middle 4 or the high 4 strings of an electric guitar set. They can be tuned to regular guitar tuning (ADGB or DGBE). But they are often tuned to open G (GDGB or DGBD) for playing with a slide. Players will sometimes say there is no wrong way to tune a CBG.
I'd say that it sounds as good as it looks. I think a lot of CBGs come equipped with single coil pickups which lend themselves to a country or folky sort of sound. In contrast, the humbucker makes this instrument mellower than you might expect and I like that a lot.
