Travis is a rather handsome and charming older fellow, with your artwork always wonderful for portraying such characters. Possibly it could be thieves looking to steal medical supplies whom tied him up, though it looks like Travis might be able to get himself free, if he can just get that scalpel and begin cutting the ropes.
