When making your plushes, make sure to trim the fur before you baste the neck.. I suspect what happened here was I trimmed the neck after (we couldn't get the joints to lock - too much material in between) and thus cut the thread or the material around it and after a little bit of tossing around the whole thing loosened right off.

This happened to me while I was working on mine. I had just finished sewing up the back and I was trying the head out. Damn thing popped right out of the neck. I'm using joints that actually require you to screw a locking nut onto them, but it was still annoying to fix when I was so close to being finished.
Why not sew one side - either the neck or head - into a disc of felt? See how the head joint is done on this to see what I mean...
