In short, this is a stylized version of Japan. All the above places are real.

These are real area names in Japan. The Kanto region is a plain east of the central mountains of Japan where currently Tokyo and large urban population reside.

Naruto is a semi-major city in the prefecture Tokushima on the island of Shikoku.

Miyazaki is a prefecture AND city on the island of Kyūshū. It's a semi-major city around 400K in population.

So often fiction will pull from real life to create it's worlds! :)

*points at Sendai* That's were one of my favorite games takes place, and in the game the characters vacation someplace in Hokkaido I believe. (Never really says where in Hokkaido in the game sadly).

*Thinks* Dude one of the anime I like also takes place in Sendai, wow almost makes me wonder if the place is a good setting fora rural/small town area since it is portrayed as such in the game and anime and they aren't related in any way.

LOL from what I can tell, Sendai is pretty built up, def not rural at any rate, also not amazingly far from where the tsunami hit... :/ Hokkaido is quite rural though... by comparison anyways.

Then the game and anime lies? rofl and yeah I know about Hokkaido a tiny bit, I have a military friend stationed in Japan that went there one day when off duty.
