You know that it's all just for legal reasons that they include all that, right? While testing the drugs on patients, anything that happens to them, regardless of whether or not it's caused by the drug, has to be recorded. I wouldn't be surprised if the "in rare cases, death" come from some tool using the drug running into the street and getting hit by a bus.
This recalls to mind a movie I saw many years ago as a kid, "Frankenstein: The True Story." In one scene, the nefarious Dr. Polidori conspires to dispose of the monster in a tank of impossibly corrosive acid. Something goes wrong (as it always does in such movies) and the monster throws one of Polidori's assistants into the tank instead, where he completely dissolves just as depicted here (albeit with a good deal more roiling and the limitations of 1970s-era effects). Never thought I'd see anything in the furry community approximating that.
