The Reason why RD sees AJ in her nightmares like this is because their relationship is "dead" therefore Bad Dream AJ looks a bit skeletal, and the multiple eyes can symbolize that RD only has eyes for AJ, and RD will try her hardest to fight to get her back until she's forced to stop.
