It is probably a play on words (in English) and expressions, since "hanging out" is an expression: informal -- to spend time relaxing or socializing informally, example use: "they're hanging out at the beach".
In this case the person in the photo is literally hanging/suspended from rope/cloth.
Expressions and puns generally do not translate very well.

That's why I didn't understand what it was about. xdd
