Kara never saw Rafael converted because when it's a red moon night, he locks her in her room and he tries to lock himself in the basement with chains.
One night Rafael, converted, freed himself and went out to hunt, but what she didn't know was that Kara had seen him leave her house without knowing why Rafael wouldn't let her go out with that phase of the moon.
Rafael was about to attack a person but before he gets to touch his new victim Kara gets in the way surprising Rafael and from what I see he knows who he is despite his transformation
