I think the fact that Billy only has one head, and a slim body, and Nurse Loupunny is slim and flat make it more...ominous. That and the evil grins.

I'll call this Universe, Universe SFW where all of Guy's characters lead healthy and normal lives.

SFW Skip: Has a successful acting career and stars in many films/tv series, enjoys eating vegetables, confident in himself, and etc.

SFW P: Sleeps normally in a normal bed and room, not into cospiracies, much more laid back that Regular P, and etc,

SFW Amal: Avoided the nuclear incident and stayed normal, talks loudly, spends a lot of his time being with people, streams himself to thousands of people, and etc.

SFW Billy: Never joined a body with his brother Jimmy (either that or the two got surgically removed from each other as babies), Donates to charities and hospitals at least once a month, enjoys hanging with his brother, etc.

SFW Jimmy: Quite possibly one of the smartest men in his world, PDCorp's chief of science, currently getting close to finding the cure for Pokerus, eats mainly fruits and vegetables, exercises frequently, showers twice a day, etc.

SFW Nurse Lopunny: Asexual, graduated top of her class at Med School, very innocent and doesn't get sex jokes, sweetest nurse you'll ever meet, etc.

Returning to this over half a year later, and neither Nurse Lopunny nor BJ have been posted (on FA) in anything new, so currently this is the most recent art of them, and thus nothing disputes that this isn't their new canon designs.
