The big secret when it comes to the rules of succession in fiction is that it works whatever way is best for the writer to get the story he wants. If the author wants, the succession could involve things like the need to be confirmed by a group of advisors appointed by the previous monarch or that the king gets to select from a shortlist and said selection is done in a way that keeps it secret until after his death. Off the king without having been publicly confirmed and/or having the political clout to ensure it's you that takes the throne and you may find yourself the business of your hated cousin's royal executioner.
