Ratara is a renowned and reputable doctor in her world, there is practically nothing difficult for her, when she wants to take a troll she doesn't hold back, besides she can correct any damage.
This image refers to a moment when she rips off one of Zedek's horns, deboxing his beak, because she knows she'll take back later, Zedek didn't like it at all, although it didn't hurt, because of Ratara's anesthesia.
