It's kind of a joke concept item for RPGs, though I suppose there have been times when the idea was actually put into practice, both in TTRPGs and video games. TV Tropes has you covered for specific examples.

Oh it's a trope. The difference here is that this chainsaw does non-lethal damage and you can succumb to it. It isn't really beneficial either since someone can use it to make someone fat, short, and a shape that is worthless.
So if anything, this chainsaw is actually a "comically exaggerated" surgeon's tool. ;b
