OK.

I'm still torn between the 'brothers' or 'lovers' theories, 'cus both make sense, but I think 'lovers' makes more sense from previous clues in the show: The interactions with Robo-Fizz, the hallucination in ep 6, which showed-> Verosika, his ex, Stolas, his current 'boyfriend', Striker, who he felt attraction to, and the REAL Fizzarolli... so it would be weird to throw a brother in there... and the whole ep 7 really, until the end it was all about love and lust, and once again Verosika, Blitzo's ex, made an appearance alongside Fizz... just in the end of the episode you can see Barbie Wire and Tilla (This last one however could have had a rol change, instead of sister she could be Barbie and Blitzo's mom, but we'll have to wait for that one too).
So yeah... I still don't know what to really think, but I believe 'ex-lovers' would be very accurate. Obviously, childhood friends to lovers and probably coworkers too since they both seemed to be into circus stuff when they were little.

The mark in their forehead tho... thats what's throwing me off. Because, if they were brothers, that would make sense since Barbie and Tilla (she may end up with another name) have it too, so it may be a hereditary mark or something... I also thought that, "Hey, Verosika got a tattoo with Blitzo's name and the she crossed it off", so maybe he could have done the same when they were dating, but he has it since childhood... so yeah... that's the weird part, 'cus all the other clues point to a past romantic relationship...

It could also be a certain group of imps thing? Like, Blitzo isn't from Wrath unlike many of the imps, and another theory is that his mark is a simbol of lust, so maybe they're imps born in the Lust Ring? Idk, I'm obviously thinking too much about it and we all would have to wait until the information is revealed, maybe next episode, maybe in season 2.

Still, I'll let it ambiguous for now.

Oh! I also would like to wait until the desing of Fizz without his costume is shown to draw him like that, but I was messign a bit with his scars and horns, 'cus another big theory is that Fizz and Blitzo end up in an accident, and that left Blitzo with scars (which another theory says that those are the white spots in an imp) and Fizz without arms, legs and probably broken horns. That would be one explanation for his robotic arms and legs.
