Let me explain:

She have genetic malformation
She inhaled too much helium as a child while playing with friends; but due to her metabolism, her body couldn't process it and go down like every other creature, so her body stayed inflated. Over the years, the condition got worse, and the helium got stored even more, and the Hyena kept growing larger. At first she was upset, but after a while the Hyena got used to it, and even learned to like it! In fact, she works as a playboy girl and she's the most fun at parties
