Pamela moaned and called to her secret lover Harley for more food. Ever since getting rooted down, she's relied on the hyena for everything, which gave her and Harley a perfect excuse to spend time together. Joker either doesn't know or, more likely, doesn't care, as he is certainly smart enough to have at least started figuring out what's going on, but too focused on his usual evildoings and eternal conflict with Batman to be concerned with what Harley and Ivy do in private. Harley's been packing Ivy's belly to its limits everyday, and now Ivy was an enormous bloated mass, becoming very round and heavy, even mutating further and sprouting more and more new mouths to eat larger amounts of food faster. And even with Ivy like this, Harley still loves her.
