Well, first you need the blank license from
flame13 to work on. You can find it at this link.
Next, you need a program that will allow you to edit a multi-layer PNG; so far not many will. (GIMP doesn't, for instance.)
For the barcode, follow this link. It makes a real, scannable barcode.
Then, put all the pieces in their respective layers, adsd a signature to the back, and Hey Presto! You have an FA license.
