I'm a pupeteer so fortunately nothing in my career had anything like this. Well, I mean there were things like the transport van when we were on tour, but that was about as safe as its possible to be.
The least safe thing in any career or hobby is always stupidity. Or general lack of awareness. I was once run over by a reversing car while performing in one of those tall booths. Apparently the driver claimed the stage jumped out of nowhere at her, along with about watching 20 kids.
