Realistically, yeah. We're told that the nanites are normally locked so they only work when activated in a lab, then stop. The ones in Otto have been hacked and that's useful for stealing the tech, but really dangerous. (Though non-replicating nanites might be OK for things like artery scrubbing.)
It's been suggested that if the nanites really do have the ability to TF people just by blood contact, people would find out and come to beg Otto for a transfusion. "I'm terminally ill -- I don't care if this turns me into a catgirl!" I might write exactly that happening, and it'd be a moral problem! Probably legal in Coalition territory only because it hasn't been banned yet.

I wasn't sure whether to call Otto "she" in this sequel. I ended up doing that because the character's obviously physically changed, but she's also not sold on the whole "female" thing. Generally I change the words based on what the character feels like, but it's not a hard and fast rule for me -- and not based on what anyone considers the Correct Way.
