In the past, Scar as a character who was under the 'kill everyone' trope and in the old drawing you can see Scar's version of himself before he changed it into that edgier version. In this new version Octavius questions why he even decided to transform himself into Scar in the first place even if he knew the consequences of what he would become...even if it was facing well, himself...
