THAT PICTURE! A few months ago I edited the original one. I made everything black and white and turned the colors of the irises bright blue and green so they would stand out. It came out amazingly and has been my desktop background for two months.
Needless to say I was stunned to see it used here!

I saw it, I've heard it, and I know what it means. I've known for a while, I just don't see how it makes sense. That's all. And English is my first language.
Look at it this way: think of the basic definitions for "and" and "how". When you put them together, it shouldn't mean what the idiom means, it should be more of a question.
