The idea behind this design is in keeping with what we have written for the Age. The big armour plates prioritise protection for the hospital sections of the Ambulance Ship. As there are any number of unscrupulous furs that would try and board an Ambulance and make off with her expensive medicines and equipment, the vessels are heavily armed with precision weapons that can blow a coilgun turret off a pirate ship a thousand kilometres off. Ambulance Ships attend to emergencies that may have been caused by said pirates, and as such do not engage unless engaged, or if the ship's patients are at risk. Even so, an Ambulance ship would aim to disable weapons, not to breach the enemy hull. The number one priority of the Ambulance at any time is the safety of the patients. The Ambulance has wings and vertical stabilisers is case of an emergency atmospheric crash landing. In any crash, attack or mishap, the section most likely to survive intact will be the armoured hospital in the midsection. The Ambulance also has a long runway, in case of some serious flight emergency in the area.

A fire in space would be a unique challenge, to say the least. It begs the question, is your 'fire engine' intended to fight fires aboard starships and stations or is it intended to assist in groundbased firefighting operations? I shiver at the thought of an orbital firehose arrangement, blasting down enough water/retardant foam to cover entire cities.
It takes months for any ship to get anywhere in the system. Space is very, very big. So an Ambulance ship will have to tend to injured, sick and healed patients for months until it can get to the next big intra-solar space station. In the mean time, it has to look out for its patients, well or unwell, against pirates or whoever else they might come across.
