Sure, they have special animations for spawning sequences and other scripted events, but anything larger than Trites/Ticks and Cherubs don't mechanically have the ability to follow you into vents or other crouch-only areas. They don't have the path-finding for it in their A.I.

Well.. even at this size Doomguy isn't completely helpless and that Imp was treating him in a friendly [if obnoxious] manner before.
I'm guessing that she's shrunken him down either for hide and seek [as some kind of stealth test] and/or to do something sexy to him.
As for why Doomguy is scared, even if they are on friendly or at least civil terms I don't think he'd appreciate being soft vored or used as a sex toy by a literal demon! He might already know she's behind him and is scared of whatever they agreed on rather than his life.

I'd imagine waaaaay stronger. Regular demons already probably eat things humans couldn't. A giant demon even more so. I don't really know the exact scientific details of stomach acid, but considering the difference in scale between the two stomachs, I'd imagine the sheer volume of acid inside her's would be a significant factor, too.
