Oh yes, that's understandable, just sticking with belly rubs is just fine. I'd want to respect the big guy's personal space, no matter how great the urge to hug that belly is, wouldn't want to make him uncomfortable.

Gosh, I can only imagine how much relief he'd feel with having someone get that part of his belly with cool or warm lotion/oil rubs.
