All the information on the right should use STENCIL STD. (standard) font, size of your choice, while the name under the picture should be Courier New Bold.

You can edit it in paint if you want, just be sure to paint white (255,255,255) over the template text. In the large blank space a picture of your fursona should go there. Any background you choose. Photoshop makes that process easier.
