I admittedly haven't read the story yet, and I've been hoping you'd include at least a brief description of what is supposed to be happening in this picture and the previous ones, but I'm assuming his creation is (finally) coming to life in that moment where he looks ready to give up and die in the fire.
Yup, you got it right on the first try.
The man had built her... but started having more and more vivid dreams... until he had a "wet dream" about her.
So he went into a seedy prohibitionist her bar to get drunk, feeling observed all the way... and definitively got drunk with... whatever was served there at the time.
Which is all but "healthy". Even more than how harmful alcohol normally is, at least.
He came back home saying "i don't care! I'mma build a way to wind her up with a vagina!" https://www.furaffinity.net/view/35032098/
Next day he realized what he had done went mad, trashed his place and ended up setting his house on fire.
Trapped and still obsessed with his paranoiac figments of imagination he is confronting his nightmares by having to wind her "like this" because he hurt his hand trying to pick up her key.
This is finally her "waking up" for the first time, having no idea what's happening and starting to take all in consideration.
